Before you begin — what these forms are
- • These are scoping intakes. Submitting one does not create a CPA-client relationship.
- • ProAxis CPA is engaged only after a written engagement letter is signed by both sides.
- • We never request SSN/ITIN, date of birth, full EIN, or government ID through these forms. Sensitive items are collected through our secure client portal once we're engaged.
- • Dollar figures are estimates for discussion. Exact amounts are reconciled from your records during the engagement.
